Instructions:
1. The controlling officer shall re-sign the column 9 by providing the number in the specified cell of the assessment.
2. The sum of the total numbers must be divided by the number of items to calculate the average number.
3. If the average number is 95 or more then special salary increase cannot be recommended. In case of promotion, the average number of the last 3 (three) years must be 70 or above.
4. If the average number is less than 50 in the evaluation of the contract period, the recruitment contract will not be renewed.
5. A report must be submitted every year (January-December). No assessment report is required for less than 3 (three) months of any year.
. The assessment report must be submitted within 3 (three) months of the end of the year, otherwise departmental action has to be taken as per rules.
. Evaluation reports shall be kept confidentially in accordance with the rules and regulations of the relevant department.
